Tuberculosis in small animals
Tuberculosis (TB) in small animals (such as companion dogs and cats, or small captive mammals) is a rare but serious granulomatous infectious disease caused primarily by members of the Mycobacterium tuberculosis complex (MTBC), including Mycobacterium tuberculosis (human strain) and Mycobacterium bovis (bovine/mammalian strain), as well as Mycobacterium microti (vole strain). Small animals typically contract the bacteria via ingestion (eating raw meat, viscera, or drinking unpasteurized milk from infected livestock) or inhalation/contact (close exposure to infected humans, wildlife reservoirs, or bite/scratch wounds).
Clinical Signs
The clinical presentation of TB in small pets is notoriously non-specific and mimics many other chronic debilitating conditions. Signs generally depend on where the granulomatous lesions form:
- Pulmonary form: Chronic, moist or hacking cough, progressive dyspnea (shortness of breath), wheezing, and low-grade pneumonia signs.
- Abdominal form: Marked and rapid weight loss, anorexia, lethargy, intermittent vomiting or diarrhea, and palpable abdominal or mesenteric masses.
- Cutaneous/Miscellaneous form: Non-healing skin ulcers, chronic draining subcutaneous abscesses (especially around the head and neck in cats), or localized lymphadenopathy.
Diagnosis
Diagnosing active TB in live companion animals is challenging because standard intradermal tuberculin tests frequently yield false-negative results in dogs and cats.
- Sampling: Vets must analyze fine-needle aspirates, biopsy tissues, or respiratory washes.
- Laboratory Testing: Confirmation relies on detecting acid-fast bacilli using Ziehl–Neelsen staining, specific Polymerase Chain Reaction (PCR) assays, and specialized Bacteriological Culture Methods.
- Blood Assays: Specialized Interferon-Gamma Release Assays (IGRA) can assist in cell-mediated immune detection.
Management and Public Health Concerns
- Treatment Complexity: While prolonged multi-antimicrobial courses (often combining three or more drugs for 6+ months) can control disease progression in individual dogs or cats under specialist care, treatment is controversial.
- Zoonotic Risks: Because MTBC agents are transmissible to humans, treating infected companion animals carries public health risks regarding resistant bacterial strain development and human exposure. Euthanasia is frequently recommended by regulatory and veterinary authorities when active mammalian or bovine TB is confirmed. In regions with rigid agricultural or zoonotic tracking, a diagnosis of M. bovis is typically a notifiable disease handled by designated state agencies like the APHA.
